Lancaster, OH – The Fairfield County Health Department is proud to announce the release of the 2026–2028 Community Health Improvement Plan (CHIP), a collaborative roadmap designed to improve the health and well-being of Fairfield County residents over the next three years.
The Community Health Improvement Plan was developed through a community-driven process that brought together healthcare providers, schools, nonprofit organizations, government agencies, businesses, and community members. Their collective expertise and commitment helped identify the county’s most pressing health challenges and establish shared priorities for action.
The four priority areas identified in the 2026–2028 Community Health Improvement Plan are:
- Access to Care
- Food Insecurity and Nutrition
- Income and Poverty (Economic Stability)
- Housing
The CHIP builds upon the findings of the Community Health Assessment and provides evidence-based strategies and measurable objectives to address the identified priorities. The plan will guide community partners as they work together to improve health outcomes, reduce disparities, and strengthen the overall health of Fairfield County.
Joe Ebel, Fairfield County Health Commissioner, added, “This plan would not have been possible without the contributions of individuals and organizations that helped complete the Community Health Assessment, and identified priorities for Community Health Improvement.”
