Fairfield County Health Department is pleased to announce that there is now an online reporting system for Fairfield County residents using a home COVID-19 home test. The reporting process is simple to use, and all information provided remains confidential.

In order to provide Fairfield County residents with the most up to date information on COVID-19 statistics, it is crucial that all data is captured in a timely manner. This self-reporting form allows the health department to better analyze case rates and identify trends.
